As noted above, the Caption is the text label that appears next to the field in the online form.
These settings determine where the Caption appears in relation to the field.
By default, the Use Group Caption Position checkbox will be selected, meaning that the caption will use the default position set for the Form Field Group. See the topic Form Field Groups: Add, Edit, Delete to review how you set that default position for the group.

However, you also have the option to set the caption position on an individual field basis.
Unselect the Use Group Caption Position checkbox. This activates the Caption Position drop-down list below.
From the Caption Position drop-down list, choose the caption's position relative to the field: Top, Bottom, Right, or Left.
